Who Can Refer

Referrals are accepted from

  • Participants

  • Families and carers

  • Local Area Coordinators (LACs)

  • Allied health professionals

  • Service providers

  • Hospitals and community services

  • Guardians and advocates

Self-referrals are also welcome.

How Referrals Work

  1. A referral is submitted by email or phone.

  2. Monarch reviews the participant’s needs and current supports.

  3. Contact is made with the participant or nominee to discuss support requirements.

  4. If appropriate, services commence and supports are coordinated in line with the participant’s NDIS plan and goals.

The aim is to respond to referrals promptly and ensure participants are connected with the right supports without unnecessary delays.

What Information Is Needed

To assist with the referral process, the following information is helpful where available

  • Participant name

  • Date of birth

  • NDIS number

  • Current plan dates

  • Type of support requested

  • Brief overview of support needs

  • Key risks or complexities

  • Contact details for participant or nominee

If some information is not available, referrals can still be discussed.
